The case under consideration is a female, 18 years old, that during the evolution of an infectious disease, has presented suddenly clinical symtoms of obnubilation, disorders in the pyramidal system and craneal nerves. The laboratory test suggested a septicemia; the EEG showed a focal lesion. The arteriographic study allowed the diagnosis of an aneurism in the middle cerebral arterial area. The surgical intervention was performed under hypothermia. The bacteriological study of an aneurysmatic sample was negative. The anatomo-pathological study showed short chains of Gram's positive coccus. Three months later the hemo-culture test showed the existence of negative coagulase staphylococcus. We emphasize the frecuency of this kind of aneurysm in the evolution of the bacterial endocarditis and also the difficulties of establishing a differential diagnosis. The higher probability of incidence occurs in the first three weeks of the evolution of the illness.
