In this paper we discuss the problems that arise in the model of measurement based on decoherence. Considering that criticisms of this model have focused on its use of the reduced states of open systems, we propose a different approach. Our proposal adopts a closed-system perspective which allows us to describe irreversible processes in systems with no interaction with an environment. Furthermore, the expectation-value approach allows us to study the evolution of quantum systems without appealing to their states. The combination of these two perspectives gives rise to a new formulation of quantum decoherence whose application to the measurement problem overcomes criticisms to the original approach.
